Your First Visit

Before your first appointment, with your permission, we’ll reach out with a confirmation message and a call to introduce ourselves and answer any initial questions you have! When you arrive, you’ll be greeted at the front desk and welcomed into a comfortable space that doesn’t feel like a typical dental office. One of our team members will take you back to an operatory, where we’ll review your concerns, take any necessary images, such as X-rays or intraoral photos, and give you a tour of your mouth to better explain your oral health. You’ll then meet with Dr. Whitaker, and together, you’ll discuss your treatment recommendations, any relevant cost quotes, and ultimately, you’ll leave with a better idea of the direction your smile is headed! We’ll also be sure to help you schedule any necessary follow-ups.
Dental Patient Forms

To help you save time in our office, you can now download and fill out our forms before you actually come to see us. Click on the link below to complete our forms in the comfort of your own home and on your own time. When the day of your appointment arrives, just bring the forms with you, and we’ll get you in to see Dr. Whitaker right away. If you have any issues accessing our forms, please let us know.
Dental Insurance

At Corbin Family Dental Care, we are happy to file your dental claims on your behalf with any privately purchased or employer-provided dental insurance. Most companies directly reimburse our office for their portion, and a few companies reimburse the patient directly. To be sure we can spend quality time with each patient, we have upgraded our status to be out-of-network with all insurance providers. This allows us to focus on delivering the absolute best care and giving our patients more choice in their treatment, without the limitations that are sometimes tied to insurance plans. That said, if you still have dental insurance, you can absolutely still use your benefits. Please let us know if you have any questions, and we’ll be happy to go over your policy with you and make sure YOU get the benefits you expect from your dental insurance company.
Brilliant Membership Plan
